In modern usage, "cretin" is a pejorative noun used to describe a person considered stupid, foolish, or obnoxious. However, the term originated as a specific medical diagnosis in the 18th century. It was used to describe individuals affected by congenital iodine deficiency syndrome (formerly called cretinism), a condition characterized by physical deformity and intellectual disability. Etymologically, it derives from the Swiss French dialectal word crestin (Christian), a term reportedly coined to remind the populace that those suffering from the condition were still human beings with souls, despite their physical and cognitive differences.

The term's acceptability is contested due to its evolution from a compassionate medical descriptor to a common insult. For many contemporary speakers, the word has undergone "semantic drift" and is fully separated from its medical roots; they view it as a generic, mild insult similar to "idiot" or "moron" (both of which also began as medical classifications) and use it without conscious intent to mock the disabled. Conversely, disability rights advocates argue that the term is an ableist slur. They contend that using a word historically associated with intellectual disability as a synonym for "stupid" or "bad" perpetuates the stigmatization of people with cognitive impairments.
